64 year old female with paroxysmal AF and COPD with type 3 PAH

64 year old woman presented to the casualty with the complaints of  Dyspnea at rest since 1 week Bilateral LL edema since 1 week  Reduced urine output since 3 days 64 year old woman, housewife by occupation who says she has been experiencing dyspnea on and off along with cough since the past 13 years for which she has been using a rotahaler ( Formoterol plus Budesonide) and was told she had bronchial asthma.  In 2016, she was admitted in an outside hospital for recurrent vomitings and pain abdomen. HALLTICKET NO:- 1601006059 30/4/21   35 year old male patient,  RESIDENT of  ramchandrapuram, OCCUPATION- works as daily wage labourer   presented with a chief complaints of 3 months  history of painless papules with erythema that started initially over the face then gradually extended to upper limb, low limbs and trunk  ~Patient got topical cream, soap and some oral drugs from nearby medical shop  ~He used them for 2 months, few lesions regressed but few persisted on nose, ear lobe, hands, legs and trunk. ~14 days ago patient developed blackish skin discolouration over hands and feet with skin peeling, cracking, ulcer formation associated with burning sensation and erythema all over the body. ~No new lesions were noted  Since 4 days patient developed continuous high grade fever associated with chills and rigors. 16/5/20 Gaddam Suhas Yadav Roll.no 53   clinical data including history, clinical findings, investigations and come up with  treatment  Following is my analysis of the patient problem. Problems are Swelling of face and abdomen  Sleep disturbances Oliguria Rashes Headaches(migraine with aura) Fatigue Swelling of face and abdomen:  she was diagnosed as G6PD deficiency  which cauese hemolytic anemia ie hemolysis of rbc  may cause right heart failure which may be the cause of shortness of breath and swelling .  Swelling of face may be caused due renal failure (oxidative stress due G6PD deficiency). This deficiency causes decreased generation ofNADPH and ATP which causes oxidative stress. These patients may have neonatal  jaundice. Hiatus hernia may cause shortness of breath.
