64 year old woman presented to the casualty with the complaints of  Dyspnea at rest since 1 week Bilateral LL edema since 1 week  Reduced urine output since 3 days 64 year old woman, housewife by occupation who says she has been experiencing dyspnea on and off along with cough since the past 13 years for which she has been using a rotahaler ( Formoterol plus Budesonide) and was told she had bronchial asthma.  In 2016, she was admitted in an outside hospital for recurrent vomitings and pain abdomen. Problems are Swelling of face and abdomen  Sleep disturbances Oliguria Rashes Headaches(migraine with aura) Fatigue Swelling of face and abdomen:  she was diagnosed as G6PD deficiency  which cauese hemolytic anemia ie hemolysis of rbc  may cause right heart failure which may be the cause of shortness of breath and swelling .  Swelling of face may be caused due renal failure (oxidative stress due G6PD deficiency). This deficiency causes decreased generation ofNADPH and ATP which causes oxidative stress. These patients may have neonatal  jaundice. Hiatus hernia may cause shortness of breath.
